Our 12th grade students have a lot to balance - keeping their grades up, post-secondary planning, extracurriculars, jobs, family responsibilities, and much more. This page is a place for our seniors to find everything they need as they move through their final year at New Foundations. From senior dues, prom and graduation information, to resources for postsecondary transition, to celebrations of their milestones and accomplishments - it can all be found here in one place.

Working Papers
Students, aged 14-18, interested in acquiring working papers should visit the Career and Technical Education Center for more information.
Senior Release of Records Form 
Each senior must have this form completed one time. Once the form is signed and completed, it should be given to the assigned counselor. Transcripts and letters of recommendation cannot be released without this form.


Senior Transcript Request Form
Every time a senior submits an application, this form must be completed and given to the assigned counselor in order for the materials to be sent. Counselors have up to 10 days from receiving the form to send materials.
